Your Initial Visit
On your first visit, a complete blood panel will be done. We will be checking your glucose level, cholesterol, triglycerides, thyroid, and getting a general wellness profile. An EKG will also be administered on your first visit.
The results will be discussed with you on your second visit. If there is a result that is dangerously out of range, the doctor will call and advise before your next scheduled appointment.
On your first visit, the doctor will prescribe a plan of treatment which may include any or all of the following: a variety of supplements, an optional vitamin B with Lipo injection, an exercise program, meal plans, and medications (phentermine, etc) when appropriate.
On your initial visit to the center we request that you make arrangements for child care. This is an important time for you and the doctor to discuss your history, and develop your personalized plan for weight loss. We have found that the most success on this visit comes with as few distractions as possible.
In order to save time filling out forms when you come to your first appointment we request that all new patients print and read all the documents and complete the forms listed here. Arlington patients, you will need to bring these completed forms to your first appointment. Dallas and Waco patients, please have these forms filled out at the time you call to make your first appointment. If you e-mail the forms in please send them to the appropriate office.
NEW PATIENTS – Please bring your drivers license or another form of picture identification to your first appointment. We will be making a copy of your identification for your file.
New Patient Charges for All Offices
Your first visit will be $190.00
Please be prepared to pay by debit, credit, money order or cash.
Fees due on day of visit
No-show Policy for New Patients
Each new patient will be required to pay a $50 non-refundable deposit at the time you make your appointment. When you come in for your appointment the $50 deposit will be applied to your first office visit. Instead of paying $190 on your first visit you’ll pay $140. If you find that it’s necessary to reschedule your appointment you must do so 2 business days (Monday – Thursday) before the date of your appointment or your $50 will be forfeited.
Your deposit will also be forfeited if you are more than 15 minutes late for your appointment and have not called to let us know you are running late. Your deposit must be made by debit or credit card, mailing or bringing in a money order, or cash. Checks will not be accepted. If you make your deposit by credit card you may schedule your appointment as soon as our staff runs your card through; if paying by money order your appointment will not be scheduled until your money order is received.
Established Patient Charges
Your monthly (every 4 weeks), recurring charge for follow-up visits will be $90.00 at our Dallas and Arlington offices and $80.00 at our Waco office.
This charge includes consultation with Doctor, a vitamin B with Lipo injection, supplements and a prescription for an appetite suppressant (diet pills) which will need to be filled at a pharmacy of your choice.
Any time a patient consults with a doctor, in-office, for any reason, the fee is $90.00 at our Dallas and Arlington offices and $80.00 at our Waco office.
Patients can come in weekly for a Vit B with Lipo injection.
(The cost for the injection will be $10.00. You will not need to have an appointment or see the doctor when you come in to get the injection.)
Established Patient No Show Policy! PLEASE READ
Recently, we have been experiencing an increased number of patients who have failed to call and cancel their appointments. In an effort to correct this problem we have found it necessary to implement a No-Show Policy for our office.
We truly regret having to implement a policy such as this, but in fairness to our patients who need to make an appointment and could have been scheduled at the time of a No-Show patient, we need to take actions necessary to see that our appointments are open and available for our patients.
Our Policy: If you miss your appointment without rescheduling at least 24 hours before the time of your appointment you will be charged a $25.00 no-show fee. If you are more than 15 minutes late it will be considered a no-show and you will need to reschedule your appointment and be charged a $25.00 no-show fee. The fee will be due when you come in for your next appointment. If you have two consecutive no-show appointments you will not be able to schedule another appointment without first paying $50.00 for the two missed appointments.
Other Useful Information
Insurance companies have historically NOT covered weight loss; however, some are becoming more receptive to the idea of treating obesity. We do not file insurance in the office but we will supply you with a Super Bill which provides you with all the information you need to file your own claim for reimbursement. ANYTIME WE ARE REQUESTED TO FILL OUT ANY TYPE OF FORM FOR YOUR INSURANCE THERE WILL BE A CHARGE. We do not have an insurance department or personnel to handle insurance issues on your behalf.
We accept cash, MasterCard, Visa, Discover, American Express and all debit cards.
Because our doctors rotate between the three offices, our hours vary greatly from office to office. Please call for appointments at the office of your choice.
